Why Is Cancer So Hard to Treat: Invasion, Metastasis, Heterogeneity, and Drug Resistance
Cancer is frightening not only because it grows, but because it changes, hides, spreads, and develops drug resistance under treatment pressure. The four hallmarks proposed by Professor Harris Busch over forty years ago remain an important gateway to understanding cancer today.
The first is invasiveness. Normal cells usually respect tissue boundaries and know where they belong. But cancer cells break through these boundaries, penetrating surrounding tissue and spreading outward like an uncontrolled root system. The second is metastasis. When cancer cells enter the bloodstream or lymphatic system, they can reach distant organs such as the lungs, liver, bones, and brain, forming new tumors. This is where many cancers become truly dangerous.
The third is heterogeneity. A tumor is not made up of a group of identical cells, but rather resembles a mixed population containing different subgroups—some grow fast, some withstand stress, some are sensitive to drugs, and some manage to survive. This explains why the same treatment can be effective in some patients but limited in others, and why a treatment that works initially may still be followed by relapse.
The fourth is drug resistance. Cancer cells can evade drug attacks through various means, such as pumping drugs out of the cell, altering the drug's target, enhancing DNA repair, or activating other survival pathways. After sensitive cancer cells are eliminated, a small number of resistant cells may remain, becoming the seeds of relapse.
Understanding these four hallmarks is not meant to discourage us, but to remind us that cancer treatment needs to be more precise, more integrative, and requires long-term management. For patients, this also means not viewing treatment as a single battle, but as a journey that requires the joint support of the medical team, family, and faith.
